Narrative:

On jul/xa/99, I replaced the GPWS computer for troubleshooting purpose since we had windshear problems in the aircraft. After finding out that the computer did not fix the problem, I proceeded to reinstall the original one back. This procedure was not documented per my supervisor's wxxz and abcd despite my request for documentation. On jul/xc/99, I was notified to attend a boi (board of inquiry) meeting on this incident, and so I did on jul/xd/99.
