Narrative:

As we climbed through 10000 ft, I noticed a smell of smoke and noticed a small amount of smoke in the cockpit. We elected to return to our point of origin to investigate the cause. Found evidence of bird ingestion into left engine. During the turnback we neglected to get the required amendment to our flight release showing milwaukee as our new destination.

Title: A FLC SMELLED THE ODOR AND SMOKE IN THE COCKPIT AND RETURNED TO MKE.

Narrative: AS WE CLBED THROUGH 10000 FT, I NOTICED A SMELL OF SMOKE AND NOTICED A SMALL AMOUNT OF SMOKE IN THE COCKPIT. WE ELECTED TO RETURN TO OUR POINT OF ORIGIN TO INVESTIGATE THE CAUSE.
