Narrative:

3 member cargo crew were operating scheduled MD11 cargo flight from hong kong to anc. Immediately prior to start-up, significant maintenance was performed on engine driven hydraulic pump system necessitating operation of the hydraulic system in manual vice normal automatic operation. This necessitated more involved crew procedures and checklists, both prior to and after start, and required extra due diligence to verify the subject system were within limits and functioning normally. Taxi instructions were received and aircraft was correctly taxied and holding short at the CAT ii holding position for runway 25L. Crew properly checked in with tower while holding short. Thereafter, the before takeoff checklist mandated further confign of the hydraulic system and crew attention to verify system was functioning properly. During this process tower advised aircraft to taxi to runway 25L holding point. Relief first officer relayed tower's call to the first officer and captain amidst the process of hydraulic system confign. First officer responded to tower '...to taxi to runway 25L holding point.' crew assumed that since we were already holding at the CAT ii holding point, and had already contacted tower, that tower instructions were obviously meant as instructions to taxi forward from our CAT ii holding position to the CAT I holding position. Crew initiated taxi after which tower shortly advised crew that tower had wanted us to hold at the CAT ii holding point. Crew halted aircraft approximately 1/2 way between CAT ii and CAT I holding point in VFR conditions and in an entirely safe position in regards to the runway and all other ground traffic.

TAXI INSTRUCTIONS WERE RECEIVED AND ACFT WAS CORRECTLY TAXIED AND HOLDING SHORT AT THE CAT II HOLDING POS FOR RWY 25L. CREW PROPERLY CHKED IN WITH TWR WHILE HOLDING SHORT. THEREAFTER, THE BEFORE TKOF CHKLIST MANDATED FURTHER CONFIGN OF THE HYD SYS AND CREW ATTN TO VERIFY SYS WAS FUNCTIONING PROPERLY. DURING THIS PROCESS TWR ADVISED ACFT TO TAXI TO RWY 25L HOLDING POINT. RELIEF FO RELAYED TWR'S CALL TO THE FO AND CAPT AMIDST THE PROCESS OF HYD SYS CONFIGN. FO RESPONDED TO TWR '...TO TAXI TO RWY 25L HOLDING POINT.' CREW ASSUMED THAT SINCE WE WERE ALREADY HOLDING AT THE CAT II HOLDING POINT, AND HAD ALREADY CONTACTED TWR, THAT TWR INSTRUCTIONS WERE OBVIOUSLY MEANT AS INSTRUCTIONS TO TAXI FORWARD FROM OUR CAT II HOLDING POS TO THE CAT I HOLDING POS. CREW INITIATED TAXI AFTER WHICH TWR SHORTLY ADVISED CREW THAT TWR HAD WANTED US TO HOLD AT THE CAT II HOLDING POINT. CREW HALTED ACFT APPROX 1/2 WAY BTWN CAT II AND CAT I HOLDING POINT IN VFR CONDITIONS AND IN AN ENTIRELY SAFE POS IN REGARDS TO THE RWY AND ALL OTHER GND TFC.
