Narrative:
Passenger was first noticed by the boarding flight attendant as drunk. She informed the gate agent, but nothing further was done. The passenger boarded the plane, and again #5 flight attendant informed me of the drunk passenger. I had the passenger removed from the flight prior to departure.

Title: FLT ATTENDANT RPT, B757, BOS-SJU, AIRLINE KNOWINGLY BOARDED INTOXICATED PAX. PURSER HAD HIM REMOVED.
Narrative: PAX WAS FIRST NOTICED BY THE BOARDING FLT ATTENDANT AS DRUNK. SHE INFORMED THE GATE AGENT, BUT NOTHING FURTHER WAS DONE. THE PAX BOARDED THE PLANE, AND AGAIN #5 FLT ATTENDANT INFORMED ME OF THE DRUNK PAX. I HAD THE PAX REMOVED FROM THE FLT PRIOR TO DEP.
