Narrative:

Aircraft xyz returned to gate, could not start #3 engine. Found starter damaged. Removed and replaced starter. The condition of the starter was not entered into the logbook. This error was discovered when abc maintenance coordinator called to request information on condition of old starter. Aircraft flown ZZZ to abc where gearbox was replaced. Maintenance coordinator called ZZZ approximately 3 hours after flight departure.

Title: AN L1011-500 WAS DISPATCHED AFTER A #3 ENG STARTER CHANGE WITH NO LOGBOOK ENTRY OF REMOVED STARTER CONDITION.

Narrative: ACFT XYZ RETURNED TO GATE, COULD NOT START #3 ENG. FOUND STARTER DAMAGED. REMOVED AND REPLACED STARTER. THE CONDITION OF THE STARTER WAS NOT ENTERED INTO THE LOGBOOK.
