Narrative:
On climb out passing 4000 ft, we saw a large bird pass by the left side of the aircraft. Immediately noticed a hum or vibration and the vibration indicator 1.7 on the left engine while the right remained near zero. As we climbed the indication rose to a peak of 2.7. A flight attendant called shortly and reported a strange noise at the left mid-cabin door. We contacted dispatch and maintenance and elected to return to ord. Landing and rollout were normal. Indications in the cockpit were at near zero vibration and a visual inspection by the fire department showed no obvious damage. Visual inspection at the gate revealed damage to 3 engine blades (fan) about 6 inches along the blade and 3 inches deep.

Title: A BOEING 757 FLC STRUCK A BIRD ON CLBOUT FROM ORD.
Narrative: ON CLBOUT PASSING 4000 FT, WE SAW A LARGE BIRD PASS BY THE L SIDE OF THE ACFT. IMMEDIATELY NOTICED A HUM OR VIBRATION AND THE VIBRATION INDICATOR 1.7 ON THE L ENG WHILE THE R REMAINED NEAR ZERO. AS WE CLBED THE INDICATION ROSE TO A PEAK OF 2.7. A FLT ATTENDANT CALLED SHORTLY AND RPTED A STRANGE NOISE AT THE L MID-CABIN DOOR. WE CONTACTED DISPATCH AND MAINT AND ELECTED TO RETURN TO ORD. LNDG AND ROLLOUT WERE NORMAL. INDICATIONS IN THE COCKPIT WERE AT NEAR ZERO VIBRATION AND A VISUAL INSPECTION BY THE FIRE DEPT SHOWED NO OBVIOUS DAMAGE. VISUAL INSPECTION AT THE GATE REVEALED DAMAGE TO 3 ENG BLADES (FAN) ABOUT 6 INCHES ALONG THE BLADE AND 3 INCHES DEEP.
