Narrative:
I have been an inspection authority/authorized for 14 yrs. I have been the director of maintenance for a corporation for 12 yrs. In aug/98 we moved our base of operations. I filed a change of address with FAA in oklahoma city but neglected to inform the FSDO of my change of base of operations. With all of the moving and uproar, I forgot about far 65.95(C) that says you must notify your new FSDO of your base of operations. I discovered the problem when I received my inspection authority/authorized renewal application from the old FSDO in the mail. I have sent my new information to the new FSDO. With all that was going on and never having changed bases before, I should have had a checklist of things to be done.

Title: A CPR DIRECTOR OF MAINT FAILS TO FILE THE NEW CPR ADDRESS WITH THE LOCAL FSDO OFFICE OF THE NEW LOCATION OF THE BASE OF OPS FOR THE CORPORATION.

WITH ALL OF THE MOVING AND UPROAR, I FORGOT ABOUT FAR 65.95(C) THAT SAYS YOU MUST NOTIFY YOUR NEW FSDO OF YOUR BASE OF OPS. I DISCOVERED THE PROB WHEN I RECEIVED MY INSPECTION AUTH RENEWAL APPLICATION FROM THE OLD FSDO IN THE MAIL. I HAVE SENT MY NEW INFO TO THE NEW FSDO. WITH ALL THAT WAS GOING ON AND NEVER HAVING CHANGED BASES BEFORE, I SHOULD HAVE HAD A CHKLIST OF THINGS TO BE DONE.
