Narrative:

After pushback and during engine start the ground crew announced all clear to taxi. During initial power up a thud from the nose gear was felt and heard. We stopped and after a short moment the tug was seen pulling away from the nose of the aircraft. The tug could not be seen from the cockpit because its flag was missing. The ground crew gave the all clear and disconnected prior to the tug being cleared. We were already frustrated by late boarding and slow ground crew response time. Just wanted to leave that hub and were rushing when incident occurred. Factors to be considered: lack of awareness -- flight crew: poor coordination. Ground crew: lack of proper procedure. Ground crew: no damage to crew or ground tug.
