Narrative:

Significant crew disharmony -- first officer unable to diffuse disharmony. Captain verbally abusive, ie, demeaning, mocking, name calling. Captain would not answer questions pertinent to flight or flight duties. First officer requested to fly in international relief officer position 3 times due to perceived inability to please captain! Request vehemently denied. First officer has professional and moral obligations to bring up information pertinent to flight and ask questions. Communications had broken down to the point that this was now not possible. First officer considers situation unsafe and removes self from cockpit. First officer then calls company operations control in the united states and speaks with supervisory pilot. First officer requests supervisory pilot to talk with captain: first officer requesting to fly in international relief officer position, for the captain to be civil to first officer and differences to be put aside for a safe, efficient flight back to the states -- and for differences to be resolved stateside. Captain agrees. In-flight, captain breaks agreement. Captain demeans first officer in front of passenger and flight attendants. Captain verbally abusive to the first officer while first officer flying in right seat (first officer working as the relief pilot for the original international relief officer). Captain refused to allow first officer access to information pertinent to flight while performing the flight duties from the right seat. Therefore, captain put transatlantic heavy jet into single pilot operation. Propose crew resource management training and counseling for captain.
