Narrative:

Aircraft X was cleared from ill to msp. Reported off and was told to report reaching 5000 ft. He did not report again and did not turn on his transponder to the assigned code until he was already in msp approach (class B) airspace. It was not noticed by 2 controllers since some strip marking was lacking. Supplemental information from acn 419159: aircraft X checked in with me. I instructed him to report reaching 5000 ft. I then departed the aircraft's flight plan. Was offered relief and in that briefing I forgot to tell the next controller about aircraft X that had departed. Aircraft X never reported reaching 5000 ft, never turned on his transponder. The next time he called center was 20 mi from msp, inside msp approach control airspace.

Title: AN IFR PA31 DEPARTS FROM AN ARPT IN ZMP AIRSPACE, ENTERING MSP APCH CTL AIRSPACE WITHOUT BEING RADAR IDENTED OR HAVING THE REQUIRED HDOF.

Narrative: ACFT X WAS CLRED FROM ILL TO MSP. RPTED OFF AND WAS TOLD TO RPT REACHING 5000 FT. HE DID NOT RPT AGAIN AND DID NOT TURN ON HIS XPONDER TO THE ASSIGNED CODE UNTIL HE WAS ALREADY IN MSP APCH (CLASS B) AIRSPACE. IT WAS NOT NOTICED BY 2 CTLRS SINCE SOME STRIP MARKING WAS LACKING. SUPPLEMENTAL INFO FROM ACN 419159: ACFT X CHKED IN WITH ME. I INSTRUCTED HIM TO RPT REACHING 5000 FT. I THEN DEPARTED THE ACFT'S FLT PLAN. WAS OFFERED RELIEF AND IN THAT BRIEFING I FORGOT TO TELL THE NEXT CTLR ABOUT ACFT X THAT HAD DEPARTED. ACFT X NEVER RPTED REACHING 5000 FT, NEVER TURNED ON HIS XPONDER. THE NEXT TIME HE CALLED CTR WAS 20 MI FROM MSP, INSIDE MSP APCH CTL AIRSPACE.
