Narrative:

Yaw damper light was written up in xax. Contract maintenance placarded yaw damper, filled out the logbook incorrectly. On subsequent flight, a PIREP of 3 units of rudder trim was necessary for straight flight. Upon arrival at ZZZ, air carrier maintenance verified the rudder was not faired to the stabilizer as required by MEL. MEL was not accomplished at xax. ZZZ air carrier maintenance accomplished MEL.

Title: A DC9 WAS DISPATCHED WITH THE YAW DAMPER 'OFF' LIGHT DEFERRED AS INOP, BUT WITHOUT THE MEL SPECIAL PROCS ACCOMPLISHED.

Narrative: YAW DAMPER LIGHT WAS WRITTEN UP IN XAX. CONTRACT MAINT PLACARDED YAW DAMPER, FILLED OUT THE LOGBOOK INCORRECTLY.
