Narrative:

Took off from tol for cak. At liftoff, an unexplained vibration occurred and the plane did not pressurize. A gradual climb was made to 5000 ft MSL to troubleshoot. An emergency was declared and the flight returned to tol. Investigation revealed the forward cargo door was open. The warning light system was inoperative so there was no way the crew could know. Plane was inspected, the door light MEL'ed and plane was returned to service.

Title: A DC9-30 HAS TO RETURN LAND AT TOL WHEN THE ACFT WILL NOT PRESSURIZE. THE ACFT WAS ALSO EXPERIENCING A VIBRATION THAT ALARMED THE CREW TO THE EXTENT THAT THEY DECLARED AN EMER. THE FORWARD BAGGAGE DOOR WAS LEFT UNLATCHED BY GND CREW.

Narrative: TOOK OFF FROM TOL FOR CAK. AT LIFTOFF, AN UNEXPLAINED VIBRATION OCCURRED AND THE PLANE DID NOT PRESSURIZE. A GRADUAL CLB WAS MADE TO 5000 FT MSL TO TROUBLESHOOT. AN EMER WAS DECLARED AND THE FLT RETURNED TO TOL. INVESTIGATION REVEALED THE FORWARD CARGO DOOR WAS OPEN. THE WARNING LIGHT SYS WAS INOP SO THERE WAS NO WAY THE CREW COULD KNOW. PLANE WAS INSPECTED, THE DOOR LIGHT MEL'ED AND PLANE WAS RETURNED TO SVC.
