Narrative:

On sep/xa/98 I received notification that a module (O-6) that I worked on in mar/98 was involved in an in-flight shutdown. After module disassembly on sep/xb/98, debris from a shop paper towel was found inside. I have no idea how the shop towel ended up on the inside of this module. The towel was located under the bevel gear housing in the tower of the rear case.

Title: A COMMERCIAL FIXED WING ACFT SHUT DOWN AN ENG DUE TO THE TWR SHAFT GEAR BOX FAILURE CAUSED BY A SHOP TOWEL LEFT IN THE GEAR BOX.

Narrative: ON SEP/XA/98 I RECEIVED NOTIFICATION THAT A MODULE (O-6) THAT I WORKED ON IN MAR/98 WAS INVOLVED IN AN INFLT SHUTDOWN. AFTER MODULE DISASSEMBLY ON SEP/XB/98, DEBRIS FROM A SHOP PAPER TOWEL WAS FOUND INSIDE. I HAVE NO IDEA HOW THE SHOP TOWEL ENDED UP ON THE INSIDE OF THIS MODULE. THE TOWEL WAS LOCATED UNDER THE BEVEL GEAR HOUSING IN THE TWR OF THE REAR CASE.
