Narrative:

The 'fever one' arrival was implemented aug/xa/98 with a crossing restr of curle at 6000 ft. This STAR was planned with turboprop aircraft maintaining ATC assigned altitude until 18 DME northeast of glen rose VOR. This distance was picked as appropriate since it was 3 mi inside approach airspace, allowing minimum separation to be maintained from aircraft within center airspace. We were briefed to this effect. On aug/xc/98, the third day of use of the fever one, while I was explaining the purpose of the STAR to a trainee, the trainee asked where it specified 'northeast of glen rose.' the note reads as follows: 'note: aircraft assigned above 6000 ft, maintain last ATC assigned altitude until glen rose 18 DME.' simply put, all of us were conditioned to expect the 'northeast' restr, so we all read it that way! Turboprops on the fever one cross into approach at 10000 ft or 9000 ft in a south flow or 7000 ft and 6000 ft in a north flow. Luckily, most of the time prior to our discovery, we were landing north. However, I clearly recall at least 2 sits, on aug/xx/98 where aircraft on the fever one crossed over traffic at the glen rose VOR at or above 6000 ft. With this error, no separation was assured!

HOWEVER, I CLRLY RECALL AT LEAST 2 SITS, ON AUG/XX/98 WHERE ACFT ON THE FEVER ONE CROSSED OVER TFC AT THE GLEN ROSE VOR AT OR ABOVE 6000 FT. WITH THIS ERROR, NO SEPARATION WAS ASSURED!
