Narrative:

Fatigue: I had been flying for 11 days straight. Very little actual flying but lots of reroutes and many very late flts. I got used to going to bed at a new time. On jun/xx/98, we were canceled on our outbound flight from knoxville and sent back to the hotel for crew rest at approximately XA00 local. I went to the room and as I was unable to sleep due to my biological rhythms being set to such a different hour I got very fatigued. Our new departure was supposed to be at XL45 but at XK30 my captain told me our flight was canceled due to no aircraft in knoxville and we were supposed to deadhead on the XP30 flight and then possibly fly some other trip. I called scheduling and told them I was fatigued and would be unable to fly after my return to atlanta. I then jump seated on an air carrier flight at XN00 to get home and get some rest. After sleeping for 4-5 hours I awoke feeling ok and called into crew scheduling for an additional trip (our airline is very very short of first officer's) and was assigned an fll flight at XA50. I was still tired slightly, but not acute fatigue, more chronic like. On climb out from atlanta we were assigned 10000 ft, then 12000 ft, then 13000 ft, which is unusual. We are normally assigned 10000 ft then directly to 14000 ft by approach. We flew right through 13000 ft to 13500 ft. Then to top it all off en route we screwed up and got off center frequency over rsw for about 10-15 mins. We had to have an approach facility find us a proper center frequency. Then we shot the wrong approach into fort lauderdale runway 9R instead of runway 9L. Contributing factors: fatigue -- due to biological rhythms being disturbed. Stress -- due to terrible management of pilot staffing creating shortfall, due to the above staffing levels being so bad necessitating frequent trip changes, due to inadequate staffing and training in crew scheduling and dispatch making above sits worse. Equipment -- aircraft are old with position deviation indicator navigation instruments, the autoflt system were very primitive (although the captain was hand flying).

AFTER SLEEPING FOR 4-5 HRS I AWOKE FEELING OK AND CALLED INTO CREW SCHEDULING FOR AN ADDITIONAL TRIP (OUR AIRLINE IS VERY VERY SHORT OF FO'S) AND WAS ASSIGNED AN FLL FLT AT XA50. I WAS STILL TIRED SLIGHTLY, BUT NOT ACUTE FATIGUE, MORE CHRONIC LIKE. ON CLBOUT FROM ATLANTA WE WERE ASSIGNED 10000 FT, THEN 12000 FT, THEN 13000 FT, WHICH IS UNUSUAL. WE ARE NORMALLY ASSIGNED 10000 FT THEN DIRECTLY TO 14000 FT BY APCH. WE FLEW RIGHT THROUGH 13000 FT TO 13500 FT. THEN TO TOP IT ALL OFF ENRTE WE SCREWED UP AND GOT OFF CTR FREQ OVER RSW FOR ABOUT 10-15 MINS. WE HAD TO HAVE AN APCH FACILITY FIND US A PROPER CTR FREQ. THEN WE SHOT THE WRONG APCH INTO FORT LAUDERDALE RWY 9R INSTEAD OF RWY 9L. CONTRIBUTING FACTORS: FATIGUE -- DUE TO BIOLOGICAL RHYTHMS BEING DISTURBED. STRESS -- DUE TO TERRIBLE MGMNT OF PLT STAFFING CREATING SHORTFALL, DUE TO THE ABOVE STAFFING LEVELS BEING SO BAD NECESSITATING FREQUENT TRIP CHANGES, DUE TO INADEQUATE STAFFING AND TRAINING IN CREW SCHEDULING AND DISPATCH MAKING ABOVE SITS WORSE. EQUIP -- ACFT ARE OLD WITH POS DEV INDICATOR NAV INSTS, THE AUTOFLT SYS WERE VERY PRIMITIVE (ALTHOUGH THE CAPT WAS HAND FLYING).
