Narrative:

First flight for aircraft after completion of 'C' check. After takeoff left gear door unsafe light remained illuminated. At 3000 ft AGL and request for 'climb power' left engine did not respond to throttle movement. Engine indications remained at 1.7 EPR, 82% N1. Additional throttle movement jammed throttle at mid-throttle range, engine remained at 1.7 EPR and 82% N1. Aircraft climbed to 10000 ft (departure clearance), emergency declared in preparation for single engine landing, crash fire rescue equipment requested. Landing gear lowered to ascertain landing gear situation (3 safe gear achieved) and to provide drag for descent. Left engine secured, straight-in approach and landing approved. Remainder of single engine approach and landing normal. Callback conversation with reporter revealed the following information: the reporter said the aircraft was a DC9-30 and the engines were P&west JT8D-9 type. The reporter stated the throttle had movement initially but then failed at mid-throttle range when climb power was needed. The reporter said the cause was the throttle mechanism connection to the fuel control throttle input lever had become disconnected due to failure to properly safety the fuel control throttle lever bolt.
