Narrative:

Through inattentiveness, both first officer and I missed the detail published on SID that the departure off of runway 28 requires turn at 3 mi to heading of 150 degrees. The SID is the 'swann two' and departure from runway 28 is the only one out of eight that is not runway heading. This is not an excuse for missing this important detail but I believe previous day's work, 6 legs, 7 hours 44 mins flight time, approach into a level 5 thunderstorm, older model of aircraft with higher workload and very early get up, contributed to our attention to detail lacking. My only suggestion to possibly ensure against further mistakes such as mine on this runway is that the tower reinforce the unique turn published on the SID when clearance for takeoff is given, eg, 'aircraft -- fly the published turn on departure, cleared for takeoff runway 28.' one final thing, we were given a departure frequency different than published. First officer had the published frequency dialed in versus one given by clearance and this caused delay and confusion on departure as crew was directed to change the frequency on departure.

Title: MLG ACFT ON SWANN TWO SID FAILED TO TURN AT 3 MI AS DEPICTED. DEP CTLR INTERVENED WITH NEW HEADING CLRNC.

Narrative: THROUGH INATTENTIVENESS, BOTH FO AND I MISSED THE DETAIL PUBLISHED ON SID THAT THE DEP OFF OF RWY 28 REQUIRES TURN AT 3 MI TO HDG OF 150 DEGS. THE SID IS THE 'SWANN TWO' AND DEP FROM RWY 28 IS THE ONLY ONE OUT OF EIGHT THAT IS NOT RWY HEADING. THIS IS NOT AN EXCUSE FOR MISSING THIS IMPORTANT DETAIL BUT I BELIEVE PREVIOUS DAY'S WORK, 6 LEGS, 7 HRS 44 MINS FLT TIME, APCH INTO A LEVEL 5 TSTM, OLDER MODEL OF ACFT WITH HIGHER WORKLOAD AND VERY EARLY GET UP, CONTRIBUTED TO OUR ATTN TO DETAIL LACKING. MY ONLY SUGGESTION TO POSSIBLY ENSURE AGAINST FURTHER MISTAKES SUCH AS MINE ON THIS RWY IS THAT THE TWR REINFORCE THE UNIQUE TURN PUBLISHED ON THE SID WHEN CLRNC FOR TKOF IS GIVEN, EG, 'ACFT -- FLY THE PUBLISHED TURN ON DEP, CLRED FOR TKOF RWY 28.' ONE FINAL THING, WE WERE GIVEN A DEP FREQ DIFFERENT THAN PUBLISHED. FO HAD THE PUBLISHED FREQ DIALED IN VERSUS ONE GIVEN BY CLRNC AND THIS CAUSED DELAY AND CONFUSION ON DEP AS CREW WAS DIRECTED TO CHANGE THE FREQ ON DEP.
