Narrative:

While initiating taxi from stand 'a' on the cargo ramp and while running the 'after start' checklist, the first officer obtained taxi clearance. Both the first officer and the captain understood, 'taxi via taxiway 6, runway 30 and hold short of runway 4R,' which is the standard taxi route during night operations. What we all failed to realize was that he cleared us, 'taxi via taxiway 6 to hold short of runway 30.' during our taxi we entered runway 30 (an active runway). The controller then requested that we expedite our taxi down runway 30 and turn left on runway 4R. After making several apologies for our actions, we were cleared for departure. Contributing factors: after a long discussion among the 3 crew members, the following are what we believe to be the contributing factors. This was the final leg of 4, of a 14 hour duty day resulting in fatigue. The first officer obtained clearance during the 'after start' checklist resulting in the other crew members being distracted. We believe complacency was also a factor due to the standard taxi route that is in effect during nighttime operations. Also, all 3 crew members believe that we heard, 'taxi via taxiway 6, runway 30 and hold short or runway 4R.' the controller may have made a mistake. Corrective actions: both the captain and the copilot write down the taxi instructions. Wait until after the 'after start' checklist to obtain taxi clearance. Verify any clearance when instructed to cross a runway when long clrncs are given.

Title: FLC OF AN ACR CARGO JET FAILED TO STOP SHORT OF A RWY DURING TAXI CAUSING THE GND CTLR TO INTERVENE AND ISSUE NEW TAXI INSTRUCTIONS.

Narrative: WHILE INITIATING TAXI FROM STAND 'A' ON THE CARGO RAMP AND WHILE RUNNING THE 'AFTER START' CHKLIST, THE FO OBTAINED TAXI CLRNC. BOTH THE FO AND THE CAPT UNDERSTOOD, 'TAXI VIA TXWY 6, RWY 30 AND HOLD SHORT OF RWY 4R,' WHICH IS THE STANDARD TAXI RTE DURING NIGHT OPS. WHAT WE ALL FAILED TO REALIZE WAS THAT HE CLRED US, 'TAXI VIA TXWY 6 TO HOLD SHORT OF RWY 30.' DURING OUR TAXI WE ENTERED RWY 30 (AN ACTIVE RWY). THE CTLR THEN REQUESTED THAT WE EXPEDITE OUR TAXI DOWN RWY 30 AND TURN L ON RWY 4R. AFTER MAKING SEVERAL APOLOGIES FOR OUR ACTIONS, WE WERE CLRED FOR DEP. CONTRIBUTING FACTORS: AFTER A LONG DISCUSSION AMONG THE 3 CREW MEMBERS, THE FOLLOWING ARE WHAT WE BELIEVE TO BE THE CONTRIBUTING FACTORS. THIS WAS THE FINAL LEG OF 4, OF A 14 HR DUTY DAY RESULTING IN FATIGUE. THE FO OBTAINED CLRNC DURING THE 'AFTER START' CHKLIST RESULTING IN THE OTHER CREW MEMBERS BEING DISTRACTED. WE BELIEVE COMPLACENCY WAS ALSO A FACTOR DUE TO THE STANDARD TAXI RTE THAT IS IN EFFECT DURING NIGHTTIME OPS. ALSO, ALL 3 CREW MEMBERS BELIEVE THAT WE HEARD, 'TAXI VIA TXWY 6, RWY 30 AND HOLD SHORT OR RWY 4R.' THE CTLR MAY HAVE MADE A MISTAKE. CORRECTIVE ACTIONS: BOTH THE CAPT AND THE COPLT WRITE DOWN THE TAXI INSTRUCTIONS. WAIT UNTIL AFTER THE 'AFTER START' CHKLIST TO OBTAIN TAXI CLRNC. VERIFY ANY CLRNC WHEN INSTRUCTED TO CROSS A RWY WHEN LONG CLRNCS ARE GIVEN.
