Narrative:

Cleared for the civet 3 arrival for runway 25L. First officer hand flying the aircraft and coping with a map shift and multiple TCASII targets. We did not get a frequency change or an approach clearance. Just past fuelr, started to leave 8000 ft and slow to 220 KTS. I told the controller we were slowing and we were given another frequency. This controller cleared us for the runway 25L approach but was concerned about who cleared us out of 8000 ft. Contributing factors: 1) high workload. 2) were not given frequency change. 3) mindset that once past arnes you are on the approach (the way it used to be -- now fuelr is the end of the civet 3). 4) map shift which occurs almost every approach even with vors manually tuned. 5) we had been held high during our arrival and were cleared for the civet finally near the point a normal approach clearance is issued. No conflicts reported.

Title: B757 ACFT ON STAR HAD MAP SHIFT. APCH CTLR FORGOT TO GIVE FLC FREQ CHANGE UNTIL FLC ADVISED THEY WERE SLOWING. THEY WERE INSIDE FUELR AND COMMENCED DSCNT ON APCH AND NEW CTLR CLRED THEM FOR THE APCH. HOWEVER, THEY SHOULD NOT HAVE STARTED DSCNT UNTIL CLRED.

CONTRIBUTING FACTORS: 1) HIGH WORKLOAD. 2) WERE NOT GIVEN FREQ CHANGE. 3) MINDSET THAT ONCE PAST ARNES YOU ARE ON THE APCH (THE WAY IT USED TO BE -- NOW FUELR IS THE END OF THE CIVET 3). 4) MAP SHIFT WHICH OCCURS ALMOST EVERY APCH EVEN WITH VORS MANUALLY TUNED. 5) WE HAD BEEN HELD HIGH DURING OUR ARR AND WERE CLRED FOR THE CIVET FINALLY NEAR THE POINT A NORMAL APCH CLRNC IS ISSUED. NO CONFLICTS RPTED.
