Narrative:

We departed the gate in dtw thinking we would get runway 21L for takeoff. After engine start ground control gave us runway 21R, a short taxi for flight attendants and us. As we approached the runway for takeoff flight attendants were not done with the safety demonstration so we pulled over to the side to let other aircraft by. Tower was not happy we did not tell him what we were doing. While all this was going on we attempted to run our checklists -- we did 'run' them but we did not check for flap position when it was called for. Flaps were not in the takeoff position so when we started our takeoff we received takeoff warning. We aborted the takeoff, put the flaps at the proper setting, reread all checklists and continued on. This was the first takeoff on day 5 of a 5 day trip. Both of us did not sleep well the night before and was probably a major cause of overlooking the flaps. Supplemental information from acn 396581: takeoff was aborted below 60 KTS. The change in expected runway causing short taxi concerns and discussions with tower while completing checklist were factors in this situation. The crew had been laying over on both coasts and proper rest was difficult to achieve. Both crew members felt fatigued. The situation would have been avoided very simply if the crew had performed better checklist discipline (read, respond, verify) and taken more control of the situation and not allowed themselves to be or feel rushed.

Title: AN LGT FLC PERFORMS AN ABORTED TKOF AT 60 KTS WHEN THE ACFT'S TKOF WARNING ALERTS CREW THAT THE WING FLAPS HAD NOT BEEN EXTENDED FOR TKOF.

Narrative: WE DEPARTED THE GATE IN DTW THINKING WE WOULD GET RWY 21L FOR TKOF. AFTER ENG START GND CTL GAVE US RWY 21R, A SHORT TAXI FOR FLT ATTENDANTS AND US. AS WE APCHED THE RWY FOR TKOF FLT ATTENDANTS WERE NOT DONE WITH THE SAFETY DEMONSTRATION SO WE PULLED OVER TO THE SIDE TO LET OTHER ACFT BY. TWR WAS NOT HAPPY WE DID NOT TELL HIM WHAT WE WERE DOING. WHILE ALL THIS WAS GOING ON WE ATTEMPTED TO RUN OUR CHKLISTS -- WE DID 'RUN' THEM BUT WE DID NOT CHK FOR FLAP POS WHEN IT WAS CALLED FOR. FLAPS WERE NOT IN THE TKOF POS SO WHEN WE STARTED OUR TKOF WE RECEIVED TKOF WARNING. WE ABORTED THE TKOF, PUT THE FLAPS AT THE PROPER SETTING, REREAD ALL CHKLISTS AND CONTINUED ON. THIS WAS THE FIRST TKOF ON DAY 5 OF A 5 DAY TRIP. BOTH OF US DID NOT SLEEP WELL THE NIGHT BEFORE AND WAS PROBABLY A MAJOR CAUSE OF OVERLOOKING THE FLAPS. SUPPLEMENTAL INFO FROM ACN 396581: TKOF WAS ABORTED BELOW 60 KTS. THE CHANGE IN EXPECTED RWY CAUSING SHORT TAXI CONCERNS AND DISCUSSIONS WITH TWR WHILE COMPLETING CHKLIST WERE FACTORS IN THIS SIT. THE CREW HAD BEEN LAYING OVER ON BOTH COASTS AND PROPER REST WAS DIFFICULT TO ACHIEVE. BOTH CREW MEMBERS FELT FATIGUED. THE SIT WOULD HAVE BEEN AVOIDED VERY SIMPLY IF THE CREW HAD PERFORMED BETTER CHKLIST DISCIPLINE (READ, RESPOND, VERIFY) AND TAKEN MORE CTL OF THE SIT AND NOT ALLOWED THEMSELVES TO BE OR FEEL RUSHED.
