Narrative:

Enclosed is a copy of the 10-9 page to help describe the incident. The situation is that we taxied through the approach path of runway 28 on taxiway a. We were given clearance to taxi to runway 28 via txwys J and a and hold short of runway 28. We had just finished deicing at the deicing bays and received our clearance. We had our checklists completed and the first officer and I had our taxi charts out (10-9 page). We taxied to runway 28 via txwys J and a to runway 28. We taxied via taxiway J and a, because of the arrangement taxiway J intercepts taxiway a and the lack of runway hold short signs for runway 28, and lack of hold short markings on taxiway a we taxied past runway 28, through the runway 28 approach path on taxiway a but never onto runway 28. Runway 28 lies approximately 200-300 ft west of taxiway a. We advise yul tower what had happened, but yul tower was not too upset or concerned because his comment (yul tower) was that you were not the first ones to do this, taking on taxiway a and forgetting to hold short of the runway 28 approach area. Yul tower advised us that there have been several incidents of this involving our carrier and several acrs. Yul tower advised to get word out at our operations about this situation. I contacted our air safety departure and advised them about the taxi situation at yul airport. They would include a NOTAM to airmen operating in and out the deicing bays at yul. I suggested to our operations that our crews exercise extreme vigilance when taxiing from the deicing bays to runway 28 via taxiway J and a at night. Because of the arrangement of the blue taxi lights are oriented it appears there is no runway or displaced threshold intersecting the taxiway. Also there is very few signs marking the runway and taxiway hold short markings to runway 28. I also notice that there are flashing amber lights at almost all other txwys intersecting the runways at yul. I would recommend that both crew members be very vigilant when taxiing at an unfamiliar airport. If we were just a little more vigilant and better signs and lights marking runway 28 at yul, we would have prevented this incident. Callback conversation with reporter revealed the following information: reporter stated that nothing has been done to help clarify the hold short marking and signage. However, since the runway 28 is usually used for departures only, and the parallel runways 24 are used for landing, the crossing of the runway 28 flight path is not as critical. He has notified his company flight management regarding this problem, but has not observed any NOTAMS of caution.

WE HAD OUR CHKLISTS COMPLETED AND THE FO AND I HAD OUR TAXI CHARTS OUT (10-9 PAGE). WE TAXIED TO RWY 28 VIA TXWYS J AND A TO RWY 28. WE TAXIED VIA TXWY J AND A, BECAUSE OF THE ARRANGEMENT TXWY J INTERCEPTS TXWY A AND THE LACK OF RWY HOLD SHORT SIGNS FOR RWY 28, AND LACK OF HOLD SHORT MARKINGS ON TXWY A WE TAXIED PAST RWY 28, THROUGH THE RWY 28 APCH PATH ON TXWY A BUT NEVER ONTO RWY 28. RWY 28 LIES APPROX 200-300 FT W OF TXWY A. WE ADVISE YUL TWR WHAT HAD HAPPENED, BUT YUL TWR WAS NOT TOO UPSET OR CONCERNED BECAUSE HIS COMMENT (YUL TWR) WAS THAT YOU WERE NOT THE FIRST ONES TO DO THIS, TAKING ON TXWY A AND FORGETTING TO HOLD SHORT OF THE RWY 28 APCH AREA. YUL TWR ADVISED US THAT THERE HAVE BEEN SEVERAL INCIDENTS OF THIS INVOLVING OUR CARRIER AND SEVERAL ACRS. YUL TWR ADVISED TO GET WORD OUT AT OUR OPS ABOUT THIS SIT. I CONTACTED OUR AIR SAFETY DEP AND ADVISED THEM ABOUT THE TAXI SIT AT YUL ARPT. THEY WOULD INCLUDE A NOTAM TO AIRMEN OPERATING IN AND OUT THE DEICING BAYS AT YUL. I SUGGESTED TO OUR OPS THAT OUR CREWS EXERCISE EXTREME VIGILANCE WHEN TAXIING FROM THE DEICING BAYS TO RWY 28 VIA TXWY J AND A AT NIGHT. BECAUSE OF THE ARRANGEMENT OF THE BLUE TAXI LIGHTS ARE ORIENTED IT APPEARS THERE IS NO RWY OR DISPLACED THRESHOLD INTERSECTING THE TXWY. ALSO THERE IS VERY FEW SIGNS MARKING THE RWY AND TXWY HOLD SHORT MARKINGS TO RWY 28. I ALSO NOTICE THAT THERE ARE FLASHING AMBER LIGHTS AT ALMOST ALL OTHER TXWYS INTERSECTING THE RWYS AT YUL. I WOULD RECOMMEND THAT BOTH CREW MEMBERS BE VERY VIGILANT WHEN TAXIING AT AN UNFAMILIAR ARPT. IF WE WERE JUST A LITTLE MORE VIGILANT AND BETTER SIGNS AND LIGHTS MARKING RWY 28 AT YUL, WE WOULD HAVE PREVENTED THIS INCIDENT. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: RPTR STATED THAT NOTHING HAS BEEN DONE TO HELP CLARIFY THE HOLD SHORT MARKING AND SIGNAGE. HOWEVER, SINCE THE RWY 28 IS USUALLY USED FOR DEPS ONLY, AND THE PARALLEL RWYS 24 ARE USED FOR LNDG, THE XING OF THE RWY 28 FLT PATH IS NOT AS CRITICAL. HE HAS NOTIFIED HIS COMPANY FLT MGMNT REGARDING THIS PROB, BUT HAS NOT OBSERVED ANY NOTAMS OF CAUTION.
