Narrative:
When the cargo loader pulled up to my aircraft the driver scraped the side of the aircraft. It caused a large area of paint scrape and a small puncture in the skin just below the door. The cargo agents informed me of what happened and after off-loading the cargo, lowered the loader so I could see the area that was hit. I was able to see inside the puncture and it appeared to me that the loader did not damage the metal inside. I decided to return to hnl with the aircraft. Upon arrival in hnl I informed maintenance via the line mechanic. I showed the mechanic the area that was scraped by the loader. After leaving the airport I realized that I failed to make the proper logbook entry. I did contact the maintenance supervisor to assure proper work/repair had been done. After returning to hnl I realize I did not go through the proper steps to bring the aircraft back.

Title: CAPT OF B737 IS NOTIFIED THAT A CARGO LOADER SCRAPED THE ACFT. THERE IS ALSO A PUNCTURE IN THE SKIN. ACFT WAS RETURNED TO HNL AND MAINT INFORMED.
Narrative: WHEN THE CARGO LOADER PULLED UP TO MY ACFT THE DRIVER SCRAPED THE SIDE OF THE ACFT. IT CAUSED A LARGE AREA OF PAINT SCRAPE AND A SMALL PUNCTURE IN THE SKIN JUST BELOW THE DOOR. THE CARGO AGENTS INFORMED ME OF WHAT HAPPENED AND AFTER OFF-LOADING THE CARGO, LOWERED THE LOADER SO I COULD SEE THE AREA THAT WAS HIT. I WAS ABLE TO SEE INSIDE THE PUNCTURE AND IT APPEARED TO ME THAT THE LOADER DID NOT DAMAGE THE METAL INSIDE. I DECIDED TO RETURN TO HNL WITH THE ACFT. UPON ARR IN HNL I INFORMED MAINT VIA THE LINE MECH. I SHOWED THE MECH THE AREA THAT WAS SCRAPED BY THE LOADER. AFTER LEAVING THE ARPT I REALIZED THAT I FAILED TO MAKE THE PROPER LOGBOOK ENTRY. I DID CONTACT THE MAINT SUPVR TO ASSURE PROPER WORK/REPAIR HAD BEEN DONE. AFTER RETURNING TO HNL I REALIZE I DID NOT GO THROUGH THE PROPER STEPS TO BRING THE ACFT BACK.
