Narrative:

Had a leak in the first officer's airspeed system. The system was troubleshot and the flex line going to the first officer indicator was left disconnected and capped. The #2 CADC was found to be the problem and was replaced. I tested the system and verified the leak was fixed and signed off that portion of the write-up which called out to repair the leak. There was no item included on the write-up to reconnect the flex line or test the #2 CADC. In flight from the abc maintenance base to def, the first officer reported erratic airspeed indication. The disconnected line was found, reconnected, and tested good. An investigation was launched.

Title: A B747-100 WAS DISPATCHED ON A FERRY FLT WITH THE FO'S AIRSPD INDICATOR NOT OPERATING DUE TO THE INDICATOR NOT CONNECTED TO THE PITOT TUBE CAUSED BY THE PITOT LINE CAPPED DURING MAINT.

Narrative: HAD A LEAK IN THE FO'S AIRSPD SYS. THE SYS WAS TROUBLESHOT AND THE FLEX LINE GOING TO THE FO INDICATOR WAS LEFT DISCONNECTED AND CAPPED. THE #2 CADC WAS FOUND TO BE THE PROB AND WAS REPLACED. I TESTED THE SYS AND VERIFIED THE LEAK WAS FIXED AND SIGNED OFF THAT PORTION OF THE WRITE-UP WHICH CALLED OUT TO REPAIR THE LEAK. THERE WAS NO ITEM INCLUDED ON THE WRITE-UP TO RECONNECT THE FLEX LINE OR TEST THE #2 CADC. IN FLT FROM THE ABC MAINT BASE TO DEF, THE FO RPTED ERRATIC AIRSPD INDICATION. THE DISCONNECTED LINE WAS FOUND, RECONNECTED, AND TESTED GOOD. AN INVESTIGATION WAS LAUNCHED.
