Narrative:

During takeoff, the upper engine cowling came off my aircraft. Based on my training and experience as an a&P mechanic, I believe that the engine cowling hinge broke. The director of maintenance said that the latches were not secured and that I did not do a proper preflight or I would have discovered that the latches were not secured. I performed my preflight as stated in the airplane flight manual. To prevent recurrence all our aircraft cowling latches will be painted 'day-glo' orange on the sides of the latches so if they are open it will be easier to see.

Title: AN ATX FREIGHTER PLT HAD AN UPPER ENG COWLING COME OFF OF THE SMT DURING TKOF. THE MAINT PERSONNEL, APPARENTLY, BELIEVE THAT THE LATCHES WERE NOT LOCKED.

Narrative: DURING TKOF, THE UPPER ENG COWLING CAME OFF MY ACFT. BASED ON MY TRAINING AND EXPERIENCE AS AN A&P MECH, I BELIEVE THAT THE ENG COWLING HINGE BROKE. THE DIRECTOR OF MAINT SAID THAT THE LATCHES WERE NOT SECURED AND THAT I DID NOT DO A PROPER PREFLT OR I WOULD HAVE DISCOVERED THAT THE LATCHES WERE NOT SECURED. I PERFORMED MY PREFLT AS STATED IN THE AIRPLANE FLT MANUAL. TO PREVENT RECURRENCE ALL OUR ACFT COWLING LATCHES WILL BE PAINTED 'DAY-GLO' ORANGE ON THE SIDES OF THE LATCHES SO IF THEY ARE OPEN IT WILL BE EASIER TO SEE.
