Narrative:

Captain changed frequency without being told by ATC. He contacted tower at srq while being vectored by tpa. We were at 4000 ft. A cherokee was holding at 3500 ft over the VOR and had to take evasive action. Later the tower cleared us to 3000 ft. After we cleared the cherokee, the tower advised us to contact tpa again. Tpa then asked the captain why he changed frequency without being told. He said he thought controller had forgotten us. No problems happened because of this matter.

Title: AN SF340 FLC LEFT APCH CTL FREQ FOR TWR FREQ BECAUSE THE CAPT THOUGHT THAT THE CTLR HAD 'FORGOTTEN' THEM. A PA28 TOOK EVASIVE ACTION TO AVOID THEM AS THE SF34O FLEW NEAR ITS HOLDING PATTERN.

TPA THEN ASKED THE CAPT WHY HE CHANGED FREQ WITHOUT BEING TOLD. HE SAID HE THOUGHT CTLR HAD FORGOTTEN US. NO PROBS HAPPENED BECAUSE OF THIS MATTER.
