Narrative:

Upon arrival of aircraft at ZZZ, the l-hand overwing slide was found missing. There was also damage to the l-hand side of the aircraft as well as a crack to the flap. Callback conversation with reporter revealed the following information: the reporter stated the slide was installed using the correct maintenance manual procedures and task documents and the latches appeared normal after door closure. The reporter advised after the compartment door was closed a check was made of the door warning system through the EICAS system and all emergency compartment doors indicated closed and locked.

Title: A B757-200 INFLT EXPERIENCED THE L OVERWING ESCAPE SLIDE COMPARTMENT DOOR OPEN AND THE SLIDE DEPLOY. THIS INCIDENT OCCURRED FIRST FLT AFTER SLIDE REPLACEMENT.

Narrative: UPON ARR OF ACFT AT ZZZ, THE L-HAND OVERWING SLIDE WAS FOUND MISSING. THERE WAS ALSO DAMAGE TO THE L-HAND SIDE OF THE ACFT AS WELL AS A CRACK TO THE FLAP. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: THE RPTR STATED THE SLIDE WAS INSTALLED USING THE CORRECT MAINT MANUAL PROCS AND TASK DOCUMENTS AND THE LATCHES APPEARED NORMAL AFTER DOOR CLOSURE. THE RPTR ADVISED AFTER THE COMPARTMENT DOOR WAS CLOSED A CHK WAS MADE OF THE DOOR WARNING SYS THROUGH THE EICAS SYS AND ALL EMER COMPARTMENT DOORS INDICATED CLOSED AND LOCKED.
