Narrative:

ATC stated to contact tower and tower will give clearance to leave 2500 ft. I heard and read back 'call tower when leaving 2500 ft.' when aircraft was about 3 NM west of mdw (in a right turn to enter base) ATC (119.35 or 119.85) called us and told us of our error. No evasive action required, no other aircraft seen. Better communications on both sides of the radio could have helped avoid this mistake. Also it is uncommon to receive such an instruction from ATC/tower. Extra caution on part of ATC should have been given.

Title: FLC FAILED TO CONTACT TWR AS INSTRUCTED BY APCH CTL DURING AN APCH RESULTING IN TWR INTERVENTION IN ORDER TO ESTABLISH CONTACT. APCH CTL FAILED TO RESPOND TO AN IMPROPER READBACK FROM THE FLC.

BETTER COMS ON BOTH SIDES OF THE RADIO COULD HAVE HELPED AVOID THIS MISTAKE. ALSO IT IS UNCOMMON TO RECEIVE SUCH AN INSTRUCTION FROM ATC/TWR. EXTRA CAUTION ON PART OF ATC SHOULD HAVE BEEN GIVEN.
