Narrative:

Upon arrival at iah it was discovered that an open item remained in the logbook from our mia departure. Prior to departure from mia I personally reviewed the log and did observe the open lavatory item. I then began my normal cockpit duties. An APU problem occurred and diverted my attention away from the open item. Supplemental information from acn 378799: departed mia with open maintenance item: air leak sink forward lavatory. Realized our mistake upon arrival at iah. With late departure, we rushed to leave gate on time and in doing so we overlooked the maintenance entry.

Title: FLC FAILS TO NOTE OPEN ITEM IN LOGBOOK REFING AN AIR LEAK IN THE LAVATORY SINK. FLC WAS DISTRACTED WITH OTHER PROBS, RUNNING LATE AND LEFT WITHOUT HAVING ITEM SIGNED OFF.

Narrative: UPON ARR AT IAH IT WAS DISCOVERED THAT AN OPEN ITEM REMAINED IN THE LOGBOOK FROM OUR MIA DEP. PRIOR TO DEP FROM MIA I PERSONALLY REVIEWED THE LOG AND DID OBSERVE THE OPEN LAVATORY ITEM. I THEN BEGAN MY NORMAL COCKPIT DUTIES. AN APU PROB OCCURRED AND DIVERTED MY ATTN AWAY FROM THE OPEN ITEM. SUPPLEMENTAL INFO FROM ACN 378799: DEPARTED MIA WITH OPEN MAINT ITEM: AIR LEAK SINK FORWARD LAVATORY. REALIZED OUR MISTAKE UPON ARR AT IAH. WITH LATE DEP, WE RUSHED TO LEAVE GATE ON TIME AND IN DOING SO WE OVERLOOKED THE MAINT ENTRY.
