Narrative:

Aircraft #1 on a missed approach/go around runway 12. Aircraft #2 departing runway 19R. Aircraft #1 did not receive landing clearance and executed go around. Aircraft #2 rotated and saw the traffic on an intercept, both aircraft descended and turned left to avoid each other. The local controller was coordinating a previous go around on runway 19R and was distraction by frequency congestion and trouble coordinating on landlines. Aircraft #2 should not have been released, local controller was not aware of the DC10 on approach to runway 12, which called 5 times to get clearance to land but to no avail. Local controller was way behind the power curve, and apparently did not realize the situation was developing. An assistant local controller or cabin attendant coordinator should have been assisting the local controller. Supplemental information from acn 375178: appeared both B737 and DC10 took evasive action at departure ends of runway 19R and runway 12. Aircraft missed by less than 1000 ft after a considerable amount of maneuvering. 2 tower frequencys in use, local control east 128.42, local control west 120.1. Operational error occurred on frequency 120.1 (local control west). Local control west was responsible for aircraft involved. I was working local control east.
