Narrative:

After I completed the overnight check, I was asked by the lead to lock out the thrust reversers and disconnect the follow-up cable. I adjusted the 'D' rod per mechanic xyz . He checked it afterwards and readjusted the rod. I was told to close the cowlings with 2 latches as they would be reopened to adjust the engine thrust reverser. XXX and I taxied the aircraft out to runway 25 for a run. XXX adjusted the EPR power on the engine at this time. While running, tower informed we needed to move so they could taxi the runway 25. We told xyz and he said he was not done but would 'adjust the stops' at the gate. After we returned to the gate, we went to the shop and conferred with the lead. I asked the lead if he needed me to stay to finish working on the aircraft and he said 'no, that the 2 low men on overtime would stay.' nothing else was asked or said. I went outside to get my tool box off the truck where xyz was and I told him that the bag for the lock-out bolts was left on the truck. While leaving for home at XX30 am, I saw the aircraft being towed to the gate.

Title: A FOKKER 100 WAS DISPATCHED WITH BOTH ENG REVERSERS LOCKED IN THE FORWARD THRUST POS FOR MAINT BUT NOT REMOVED PRIOR TO FLT.

Narrative: AFTER I COMPLETED THE OVERNIGHT CHK, I WAS ASKED BY THE LEAD TO LOCK OUT THE THRUST REVERSERS AND DISCONNECT THE FOLLOW-UP CABLE. I ADJUSTED THE 'D' ROD PER MECH XYZ . HE CHKED IT AFTERWARDS AND READJUSTED THE ROD. I WAS TOLD TO CLOSE THE COWLINGS WITH 2 LATCHES AS THEY WOULD BE REOPENED TO ADJUST THE ENG THRUST REVERSER. XXX AND I TAXIED THE ACFT OUT TO RWY 25 FOR A RUN. XXX ADJUSTED THE EPR PWR ON THE ENG AT THIS TIME. WHILE RUNNING, TWR INFORMED WE NEEDED TO MOVE SO THEY COULD TAXI THE RWY 25. WE TOLD XYZ AND HE SAID HE WAS NOT DONE BUT WOULD 'ADJUST THE STOPS' AT THE GATE. AFTER WE RETURNED TO THE GATE, WE WENT TO THE SHOP AND CONFERRED WITH THE LEAD. I ASKED THE LEAD IF HE NEEDED ME TO STAY TO FINISH WORKING ON THE ACFT AND HE SAID 'NO, THAT THE 2 LOW MEN ON OVERTIME WOULD STAY.' NOTHING ELSE WAS ASKED OR SAID. I WENT OUTSIDE TO GET MY TOOL BOX OFF THE TRUCK WHERE XYZ WAS AND I TOLD HIM THAT THE BAG FOR THE LOCK-OUT BOLTS WAS LEFT ON THE TRUCK. WHILE LEAVING FOR HOME AT XX30 AM, I SAW THE ACFT BEING TOWED TO THE GATE.
