Narrative:

ATC facility: washington approach. Location: armel/026/8. Approach cleared us from 15000 ft to 12000 ft. The first officer started a normal descent. Out of about 12500 ft approach asked us if we had stopped our descent at 13000 ft. Our reply was, that we were cleared to 12000 ft. Approach control then cleared us to 12000 ft. Our crew procedures were, the captain set the altitude alert at 12000 ft and I as first officer confirmed the 12000 ft was set in the window.
