Narrative:

After crossing whigg intersection as depicted on the 10-3 arrival chart, ATC cleared us to descend to 10000 ft. Shortly afterward ATC issued a traffic call: 'traffic 12 O'clock, 3 mi, 9500 ft climbing. Climb as required.' simultaneously, TCASII alerted on the target. TCASII confirmed ATC traffic report -- a virtual head on conflict existed with rapid rate of closure. PF began an immediate arrest of the descent rate and initiated a climb. The last time I glanced at the TCASII readout, there was only 300 ft of vertical separation. As I looked up, the aircraft came into view. (A twin engined GA aircraft -- possibly a king air.) a further 'pull' was the only thing that kept the 2 aircraft from 'trading paint.' obviously, separation had lapsed to less than that dictated by safety and prudence. Though no passenger injuries resulted, the #2 flight attendant sustained broken ankles and shoulder. Suggestion: all aircraft capable of 250 KT cruise should be required to file and operate under IFR. No VFR for turboprops or turbojets. None! Far part 135 operators who allow their pilots to cancel IFR in order to go 'VFR direct' should be cited for 'careless and reckless' operations.

Title: B737-300 ACFT ON ARR STAR RECEIVED TFC ALERT FROM APCH CTLR AT THE SAME TIME TCASII RA. FLC FOLLOWED RA WITH HARD PULL UP RESULTING IN FLT ATTENDANT INJURY. INTRUDER TFC HAD NO ALT READOUT SO CTLR HAD NO ALT INFO, NOR IN RADIO CONTACT WITH INTRUDER.
