Narrative:

On apr xb, 1997, advised that on apr X,a 1997, on aircraft I worked had a slat disagree light. After investigation, #4 slat adjustment bolts were not cotter keyed. Callback conversation with reporter revealed the following information: the slat was installed per the maintenance manual and the first flight after installation the slat disagree warning light was illuminated continuously. Down-line station found the slat adjustment bolts loose and no cotter pins installed in the slat. The reporter states when installing the slat no mismatch occurred so it was not necessary to readjust the main track adjustment bolts. The maintenance manual stated the bolts were already torqued and the cotter pins installed on a serviceable slat. The shop that overhauled and made the slat serviceable had the practice of releasing the slat for installation with the main track adjustment bolts loose and the cotter pins not installed. The shop noted this deviation with a tag attached to the slat. No tag was seen by either mechanic or the presence of the tag and slat confign noted in the maintenance manual. The mismatched slat was the cause of the slat disagree warning light on continuously.

Narrative: ON APR XB, 1997, ADVISED THAT ON APR X,A 1997, ON ACFT I WORKED HAD A SLAT DISAGREE LIGHT. AFTER INVESTIGATION, #4 SLAT ADJUSTMENT BOLTS WERE NOT COTTER KEYED. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: THE SLAT WAS INSTALLED PER THE MAINT MANUAL AND THE FIRST FLT AFTER INSTALLATION THE SLAT DISAGREE WARNING LIGHT WAS ILLUMINATED CONTINUOUSLY. DOWN-LINE STATION FOUND THE SLAT ADJUSTMENT BOLTS LOOSE AND NO COTTER PINS INSTALLED IN THE SLAT. THE SHOP THAT OVERHAULED AND MADE THE SLAT SERVICEABLE HAD THE PRACTICE OF RELEASING THE SLAT FOR INSTALLATION WITH THE MAIN TRACK ADJUSTMENT BOLTS LOOSE AND THE COTTER PINS NOT INSTALLED. THE SHOP NOTED THIS DEV WITH A TAG ATTACHED TO THE SLAT. NO TAG WAS SEEN BY EITHER MECH OR THE PRESENCE OF THE TAG AND SLAT CONFIGN NOTED IN THE MAINT MANUAL. THE MISMATCHED SLAT WAS THE CAUSE OF THE SLAT DISAGREE WARNING LIGHT ON CONTINUOUSLY.
