Narrative:

On mar/wed/97 the aircraft departed mem without the emergency escape slide installed in the main cabin door. It was not annotated in the aircraft maintenance log. I should have mentioned this to the captain before pushback but did not until we were airborne. I should have used my CRM skills to have not allowed this to happen. Due to my inexperience I allowed myself to become rushed which led to my failure of not mentioning the slide to the captain.

Narrative: ON MAR/WED/97 THE ACFT DEPARTED MEM WITHOUT THE EMER ESCAPE SLIDE INSTALLED IN THE MAIN CABIN DOOR. IT WAS NOT ANNOTATED IN THE ACFT MAINT LOG.
