Narrative:

Flight left out of den with an incorrect deferred item on the release. Maintenance in den did not change the placard in the computer to the correct placard and captain left out of den without a release showing correct MEL item. Flight safety was not jeopardized, but there was a definite lack of communication between maintenance, dispatch, and the flight crew.

Title: WDB DEPARTED WITH INCORRECT DEFERRED ITEM ON DISPATCH RELEASE. MAINT HAD NOT ENTERED THE PROPER ITEM IN THE COMPUTER. THE TRIP WAS LEGAL WITH THE DEFERRED ITEM. SAFETY WAS NOT COMPROMISED BUT THE PAPERWORK WAS INCORRECT.

Narrative: FLT LEFT OUT OF DEN WITH AN INCORRECT DEFERRED ITEM ON THE RELEASE. MAINT IN DEN DID NOT CHANGE THE PLACARD IN THE COMPUTER TO THE CORRECT PLACARD AND CAPT LEFT OUT OF DEN WITHOUT A RELEASE SHOWING CORRECT MEL ITEM. FLT SAFETY WAS NOT JEOPARDIZED, BUT THERE WAS A DEFINITE LACK OF COM BTWN MAINT, DISPATCH, AND THE FLC.
