Narrative:

During climb out, audible bang, aircraft shudder felt and right engine indications of decreasing N1, N2, oil quantity. In-flight shutdown accomplished as per pilots operations manual. Human factors: slight communication breakdown during this time critical abnormal. After landing, at the gate, the flight attendant mentioned that 2 passenger on right side of aircraft saw engine torching and debris exiting the malfunctioning engine. This vital information could have resulted in different flight crew decisions. Consideration would be given to discharging the fire bottle even though there was no cockpit indications. This human factors breakdown could be prevented by a more thorough preflight, flight attendant briefing and in-flight. During the abnormal, request the csc to state any observations from the cabin area.

HUMAN FACTORS: SLIGHT COM BREAKDOWN DURING THIS TIME CRITICAL ABNORMAL. AFTER LNDG, AT THE GATE, THE FLT ATTENDANT MENTIONED THAT 2 PAX ON R SIDE OF ACFT SAW ENG TORCHING AND DEBRIS EXITING THE MALFUNCTIONING ENG. THIS VITAL INFO COULD HAVE RESULTED IN DIFFERENT FLC DECISIONS. CONSIDERATION WOULD BE GIVEN TO DISCHARGING THE FIRE BOTTLE EVEN THOUGH THERE WAS NO COCKPIT INDICATIONS. THIS HUMAN FACTORS BREAKDOWN COULD BE PREVENTED BY A MORE THOROUGH PREFLT, FLT ATTENDANT BRIEFING AND INFLT. DURING THE ABNORMAL, REQUEST THE CSC TO STATE ANY OBSERVATIONS FROM THE CABIN AREA.
