Narrative:

I was the first officer on air carrier flight ab from iah to msy on oct xx 1996. Departure time was approximately XA30 CST. We elected to return to iah after takeoff when the aircraft failed to pressurize because the east&east compartment door opened during the takeoff roll. Approaching V1 speed, I felt a pressurization surge in my ears. About the time I looked up at the pressurization panel, the captain said, check the door lights. I looked down to see that the east&east compartment door light had come on. After rotation, the aircraft would not pressurize and the east&east compartment door light was on steady. The flight was an hour late departing because the inbound crew had written up the pressurization. Maintenance replaced both outflow control valve motors. When they finished, the mechanic brought us the logbook and said everything was closed up and signed off. Prior to push back, the ground crew stated all doors and panels were secured and about that time I observed the captain reach over and push the annunciator and digital light test button to check that the door lights were out. The door lights were again confirmed out on the after start checklist and the taxi checklist when I checked the flight controls. Prior to taking the runway, the annunciator panel was again checked during the before takeoff checklist and there were no door lights on. In at least 4 checks of the annunciator panel prior to takeoff, the door lights were confirmed out. I believe the east&east compartment door was closed at the gate, but not properly secured and vibrated open on the takeoff roll.

Title: MD80 ACFT IS UNABLE TO PRESSURIZE AFTER TKOF FROM IAH. E&E COMPARTMENT DOOR WAS NOT FIRMLY SECURED.

Narrative: I WAS THE FO ON ACR FLT AB FROM IAH TO MSY ON OCT XX 1996. DEP TIME WAS APPROX XA30 CST. WE ELECTED TO RETURN TO IAH AFTER TKOF WHEN THE ACFT FAILED TO PRESSURIZE BECAUSE THE E&E COMPARTMENT DOOR OPENED DURING THE TKOF ROLL. APCHING V1 SPD, I FELT A PRESSURIZATION SURGE IN MY EARS. ABOUT THE TIME I LOOKED UP AT THE PRESSURIZATION PANEL, THE CAPT SAID, CHK THE DOOR LIGHTS. I LOOKED DOWN TO SEE THAT THE E&E COMPARTMENT DOOR LIGHT HAD COME ON. AFTER ROTATION, THE ACFT WOULD NOT PRESSURIZE AND THE E&E COMPARTMENT DOOR LIGHT WAS ON STEADY. THE FLT WAS AN HR LATE DEPARTING BECAUSE THE INBOUND CREW HAD WRITTEN UP THE PRESSURIZATION. MAINT REPLACED BOTH OUTFLOW CTL VALVE MOTORS. WHEN THEY FINISHED, THE MECH BROUGHT US THE LOGBOOK AND SAID EVERYTHING WAS CLOSED UP AND SIGNED OFF. PRIOR TO PUSH BACK, THE GND CREW STATED ALL DOORS AND PANELS WERE SECURED AND ABOUT THAT TIME I OBSERVED THE CAPT REACH OVER AND PUSH THE ANNUNCIATOR AND DIGITAL LIGHT TEST BUTTON TO CHK THAT THE DOOR LIGHTS WERE OUT. THE DOOR LIGHTS WERE AGAIN CONFIRMED OUT ON THE AFTER START CHKLIST AND THE TAXI CHKLIST WHEN I CHKED THE FLT CTLS. PRIOR TO TAKING THE RWY, THE ANNUNCIATOR PANEL WAS AGAIN CHKED DURING THE BEFORE TKOF CHKLIST AND THERE WERE NO DOOR LIGHTS ON. IN AT LEAST 4 CHKS OF THE ANNUNCIATOR PANEL PRIOR TO TKOF, THE DOOR LIGHTS WERE CONFIRMED OUT. I BELIEVE THE E&E COMPARTMENT DOOR WAS CLOSED AT THE GATE, BUT NOT PROPERLY SECURED AND VIBRATED OPEN ON THE TKOF ROLL.
