Narrative:

In sum: on sep/xx/96, reporter was the so on flight from sel to osa. Airplane stayed overnight at sel. During the cockpit setup, the so failed to notice that maintenance had positioned the pressurization mode selector switch from automatic to manual. After takeoff the outflow valves remained open and the airplane would not pressurize. The climb was very rapid due to the light aircraft and short flight time. Passing through 10000 ft the cabin warning horn failed to sound. As the aircraft reached 14000 ft the oxygen warning light came on and all the seat oxygen masks deployed. Aircraft was leveled off and pressurization system was selected to automatic mode. The captain advised the flight attendants and passenger that the situation was corrected. It was later found that the cabin altitude warning system was inoperative and repaired at osa.

Title: A WDB DEPARTED WITH THE PRESSURIZATION OUTFLOW VALVES FULL OPEN, AND WHEN PASSING THROUGH 14000 FT, THE PAX EMER OXYGEN MASKS DEPLOYED.

Narrative: IN SUM: ON SEP/XX/96, RPTR WAS THE SO ON FLT FROM SEL TO OSA. AIRPLANE STAYED OVERNIGHT AT SEL. DURING THE COCKPIT SETUP, THE SO FAILED TO NOTICE THAT MAINT HAD POSITIONED THE PRESSURIZATION MODE SELECTOR SWITCH FROM AUTO TO MANUAL. AFTER TKOF THE OUTFLOW VALVES REMAINED OPEN AND THE AIRPLANE WOULD NOT PRESSURIZE. THE CLB WAS VERY RAPID DUE TO THE LIGHT ACFT AND SHORT FLT TIME. PASSING THROUGH 10000 FT THE CABIN WARNING HORN FAILED TO SOUND. AS THE ACFT REACHED 14000 FT THE OXYGEN WARNING LIGHT CAME ON AND ALL THE SEAT OXYGEN MASKS DEPLOYED. ACFT WAS LEVELED OFF AND PRESSURIZATION SYS WAS SELECTED TO AUTO MODE. THE CAPT ADVISED THE FLT ATTENDANTS AND PAX THAT THE SIT WAS CORRECTED. IT WAS LATER FOUND THAT THE CABIN ALT WARNING SYS WAS INOP AND REPAIRED AT OSA.
