Narrative:

On a flight from gox to anc I was the alternate flight crew member (afcm) onboard to help captain and first officer with duties and if they need a break. Entering united states airspace near provideniya bay, russia, we made contact with USA controller and were given a clearance of direct mcg, the tager 2 arrival, anc. The first officer read back the clearance. I was sitting in the captain's seat acting as the PF and the captain was resting back in first class. We were approximately 900 NM from mcg when we received that clearance. Sometime later the captain returned to retake his left seat and I went to the back. This occurred prior to mcg. Normally we are cleared J111 from mcg to anc. The tager arrival is the same heading outbound as J111 (100 degrees) for 80 NM then a 89 degree heading to approach anc from the north. I came back to the cockpit at approximately 100 NM from mcg in time to hear zan say 'XXX are you flying the tager 2 arrival?' at this point the captain looked at the first officer and said, 'I thought we were cleared J111!' the first officer couldn't remember the clearance and I added that we had been cleared via the tager 2 when we entered united states airspace 900 mi ago! ATC was upset and gave us an immediate vector and stopped our descent at FL190. He told us that he had another aircraft climbing on J111 through the 10000 ft that we had been cleared to. That aircraft was leveled off at approximately 16000 ft until we exited the airway. Obviously we all felt terrible as the potential for an accident had occurred. Neither of us wrote down the new clearance (tager 2 arrival) back at provideniya bay, russia, when we received it. Why? Discussion afterwards seemed was because we seemed so far away from anc at the time. I would have gotten the plate out, and should have anyway, even though none of my plates were near me. It was the other captain's navigation kit! I assumed the first officer was going to tell the acting captain the new clearance but he forgot it. The acting captain flew the flight plan clearance. Lesson: from now on, whenever flying in a 3 pilot operation (something new for our carrier) all pilots must brief each other on what has occurred while one pilot is out of the cockpit, ie, new WX, amended clrncs, etc. One quick note saying 'tager 2' left on the captain's yoke clipboard would have solved everything!

Title: CAPT RESTING IN BACK OF ACFT. RELIEF PLT IN CAPT SEAT. FO READS BACK CLRNC, DIRECT MCG, TAGER 2 ARR WHEN 900 MI FROM MCG. FAILED TO PASS NEW ROUTING TO CAPT UPON HIS RETURN TO COCKPIT. FLEW FLT PLAN J111 RESULTING IN CONFLICT WITH ACFT CLBING ON J111. CLRED TO STOP DSCNT FL190 UNTIL CLR OF ACFT MAINTAINING 16000 FT.

Narrative: ON A FLT FROM GOX TO ANC I WAS THE ALTERNATE FLC MEMBER (AFCM) ONBOARD TO HELP CAPT AND FO WITH DUTIES AND IF THEY NEED A BREAK. ENTERING UNITED STATES AIRSPACE NEAR PROVIDENIYA BAY, RUSSIA, WE MADE CONTACT WITH USA CTLR AND WERE GIVEN A CLRNC OF DIRECT MCG, THE TAGER 2 ARR, ANC. THE FO READ BACK THE CLRNC. I WAS SITTING IN THE CAPT'S SEAT ACTING AS THE PF AND THE CAPT WAS RESTING BACK IN FIRST CLASS. WE WERE APPROX 900 NM FROM MCG WHEN WE RECEIVED THAT CLRNC. SOMETIME LATER THE CAPT RETURNED TO RETAKE HIS L SEAT AND I WENT TO THE BACK. THIS OCCURRED PRIOR TO MCG. NORMALLY WE ARE CLRED J111 FROM MCG TO ANC. THE TAGER ARR IS THE SAME HDG OUTBOUND AS J111 (100 DEGS) FOR 80 NM THEN A 89 DEG HDG TO APCH ANC FROM THE N. I CAME BACK TO THE COCKPIT AT APPROX 100 NM FROM MCG IN TIME TO HEAR ZAN SAY 'XXX ARE YOU FLYING THE TAGER 2 ARR?' AT THIS POINT THE CAPT LOOKED AT THE FO AND SAID, 'I THOUGHT WE WERE CLRED J111!' THE FO COULDN'T REMEMBER THE CLRNC AND I ADDED THAT WE HAD BEEN CLRED VIA THE TAGER 2 WHEN WE ENTERED UNITED STATES AIRSPACE 900 MI AGO! ATC WAS UPSET AND GAVE US AN IMMEDIATE VECTOR AND STOPPED OUR DSCNT AT FL190. HE TOLD US THAT HE HAD ANOTHER ACFT CLBING ON J111 THROUGH THE 10000 FT THAT WE HAD BEEN CLRED TO. THAT ACFT WAS LEVELED OFF AT APPROX 16000 FT UNTIL WE EXITED THE AIRWAY. OBVIOUSLY WE ALL FELT TERRIBLE AS THE POTENTIAL FOR AN ACCIDENT HAD OCCURRED. NEITHER OF US WROTE DOWN THE NEW CLRNC (TAGER 2 ARR) BACK AT PROVIDENIYA BAY, RUSSIA, WHEN WE RECEIVED IT. WHY? DISCUSSION AFTERWARDS SEEMED WAS BECAUSE WE SEEMED SO FAR AWAY FROM ANC AT THE TIME. I WOULD HAVE GOTTEN THE PLATE OUT, AND SHOULD HAVE ANYWAY, EVEN THOUGH NONE OF MY PLATES WERE NEAR ME. IT WAS THE OTHER CAPT'S NAV KIT! I ASSUMED THE FO WAS GOING TO TELL THE ACTING CAPT THE NEW CLRNC BUT HE FORGOT IT. THE ACTING CAPT FLEW THE FLT PLAN CLRNC. LESSON: FROM NOW ON, WHENEVER FLYING IN A 3 PLT OP (SOMETHING NEW FOR OUR CARRIER) ALL PLTS MUST BRIEF EACH OTHER ON WHAT HAS OCCURRED WHILE ONE PLT IS OUT OF THE COCKPIT, IE, NEW WX, AMENDED CLRNCS, ETC. ONE QUICK NOTE SAYING 'TAGER 2' LEFT ON THE CAPT'S YOKE CLIPBOARD WOULD HAVE SOLVED EVERYTHING!
