Oceanic location, 52N 020W the first flight attendant informed me that the #9 flight attendant passed out. At the time of the incident we were established on the nat system. 2 physicians were attending to the ill flight attendant and they informed me that she may have a ruptured appendix and recommend that we land at the nearest airport which happened to be snn. At 020W we contacted shanwick oceanic and declared a medical emergency, followed the procedures for exiting the nat tracks, turned 90 degrees to a heading of 360 degrees and started to descend to FL360. Once cleared to snn we contacted ftw dispatch via radio phone patch. Then reviewed procedures for overweight landing. The landing weight was 298700 pounds.
