Narrative:

Cargo only hazmat was loaded 1 min prior to push. I signed our company's form for hazmat notification. It appeared completed and legal. The 'cargo only aircraft' was circled which I missed because I was used to another form that has cargo only across entire page. There was no incident or spillage. I was unable to see box prior to departure as is normal because of late arrival on flight deck. FAA inspector did not notice this discrepancy either. He was riding in the jump seat.

Title: DURING AN FAA LINE CHK, AN ACR FLC ACCEPTED 'CARGO FLT ONLY' HAZMAT ON BOARD A PAX CARRYING FLT. THE CREW RPTS THAT THE FORM WAS NEW AND STATES THAT THE CARGO ONLY LIMITATION SHOULD BE MORE PROMINENT.

Narrative: CARGO ONLY HAZMAT WAS LOADED 1 MIN PRIOR TO PUSH. I SIGNED OUR COMPANY'S FORM FOR HAZMAT NOTIFICATION. IT APPEARED COMPLETED AND LEGAL. THE 'CARGO ONLY ACFT' WAS CIRCLED WHICH I MISSED BECAUSE I WAS USED TO ANOTHER FORM THAT HAS CARGO ONLY ACROSS ENTIRE PAGE. THERE WAS NO INCIDENT OR SPILLAGE. I WAS UNABLE TO SEE BOX PRIOR TO DEP AS IS NORMAL BECAUSE OF LATE ARR ON FLT DECK. FAA INSPECTOR DID NOT NOTICE THIS DISCREPANCY EITHER. HE WAS RIDING IN THE JUMP SEAT.
