Narrative:

Flight stl-pah discovered in-flight that captain's name on the release was incorrect. Everything else was correct on the release. Unintentional and a joint captain/dispatcher/crew scheduler responsibility. This occurrence was due to rushed circumstances to allow an on-time departure. The captain needs to take the time necessary to examine obscure parts of the release. The dispatcher needs to do the same. The crew schedulers need to stop switching reserve assignments at the last min. The release could be formatted better than it currently is.

Title: CAPT NAME ON RELEASE WAS INCORRECT, BUT NOT DISCOVERED UNTIL THE FLC WAS AIRBORNE. RPTR ATTRIBUTES THE OVERSIGHT TO RUSHED CIRCUMSTANCES FOR AN ON TIME DEP.

Narrative: FLT STL-PAH DISCOVERED INFLT THAT CAPT'S NAME ON THE RELEASE WAS INCORRECT. EVERYTHING ELSE WAS CORRECT ON THE RELEASE. UNINTENTIONAL AND A JOINT CAPT/DISPATCHER/CREW SCHEDULER RESPONSIBILITY. THIS OCCURRENCE WAS DUE TO RUSHED CIRCUMSTANCES TO ALLOW AN ON-TIME DEP. THE CAPT NEEDS TO TAKE THE TIME NECESSARY TO EXAMINE OBSCURE PARTS OF THE RELEASE. THE DISPATCHER NEEDS TO DO THE SAME. THE CREW SCHEDULERS NEED TO STOP SWITCHING RESERVE ASSIGNMENTS AT THE LAST MIN. THE RELEASE COULD BE FORMATTED BETTER THAN IT CURRENTLY IS.
