Narrative:

I was the PF and was distracted by setting up my arrival. The clearance received was 'cross 30 south of yardley at 12000 ft.' the PNF repeated the clearance to the controller. I asked what he had cleared us to. The PNF said to me 'cross yardley at 12000 ft.' at 28 DME the controller stated we needed to be at 12000 ft. We should be a lot more careful when transferring critical information from one pilot to the other. Perhaps if one pilot did not hear the clearance, confirmation should be asked for from the controller. Callback conversation with reporter revealed the following information: reporter first officer was called to confirm the date of occurrence because the tear sheet was in error. Reporter confirmed the date and also noted that they had a breakdown in their cockpit communication. Why the PNF gave the wrong clearance to the reporter is not known.

Narrative: I WAS THE PF AND WAS DISTRACTED BY SETTING UP MY ARR. THE CLRNC RECEIVED WAS 'CROSS 30 S OF YARDLEY AT 12000 FT.' THE PNF REPEATED THE CLRNC TO THE CTLR. I ASKED WHAT HE HAD CLRED US TO. THE PNF SAID TO ME 'CROSS YARDLEY AT 12000 FT.' AT 28 DME THE CTLR STATED WE NEEDED TO BE AT 12000 FT. WE SHOULD BE A LOT MORE CAREFUL WHEN TRANSFERRING CRITICAL INFO FROM ONE PLT TO THE OTHER. PERHAPS IF ONE PLT DID NOT HEAR THE CLRNC, CONFIRMATION SHOULD BE ASKED FOR FROM THE CTLR. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: RPTR FO WAS CALLED TO CONFIRM THE DATE OF OCCURRENCE BECAUSE THE TEAR SHEET WAS IN ERROR. RPTR CONFIRMED THE DATE AND ALSO NOTED THAT THEY HAD A BREAKDOWN IN THEIR COCKPIT COM. WHY THE PNF GAVE THE WRONG CLRNC TO THE RPTR IS NOT KNOWN.
