Narrative:

During my time with my current company I failed to write down my total monthly hours. I thought it meant time in last 30-31 days on the form which doesn't apply to duty records. However, it was explained to me by the director of operations that it is total flight time from the beginning of the calendar month. The mistake was also due to training that wasn't specific and didn't include any examples.
