Narrative:

Climbing through 12000 ft we noticed that there was a problem with the pressurization. The captain asked to descend to 10000 ft. It turned out that the air-conditioning supply switches were in the off position. The switches were turned on and the aircraft was pressurized. The flight continued without incidence. There are several factors which contributed to this event. The air-conditioning switches were turned on after the engines were started and turned off so that the aircraft could be deiced while on the ground. After the deice procedure the switches were not turned back on. During the deice procedure I was trying to get our weight and balance data from load control. It turned out that they were using the wrong aircraft number and that is why we were unable to receive the weight and balance over the ACARS. I checked the flight release and it had the wrong aircraft number on it. As we taxied from the deice pad, I called system control to get an amended release. While talking to system, the captain kept telling me what to say and seemed agitated. He then got on the radio and said that he needed an amended release immediately and that we were #1 for takeoff. They gave him a verbal release on the radio. The before takeoff checklist was completed and we were cleared for takeoff. The reconfign checklist was not completed after the deice procedure. There was a lot of confusion.

Title: FLC INDUCED PRESSURIZATION PROB. FORGOT TO TURN ON AIR- CONDITIONING SUPPLY SWITCHES AFTER BEING DEICED ON THE GND.

Narrative: CLBING THROUGH 12000 FT WE NOTICED THAT THERE WAS A PROB WITH THE PRESSURIZATION. THE CAPT ASKED TO DSND TO 10000 FT. IT TURNED OUT THAT THE AIR-CONDITIONING SUPPLY SWITCHES WERE IN THE OFF POS. THE SWITCHES WERE TURNED ON AND THE ACFT WAS PRESSURIZED. THE FLT CONTINUED WITHOUT INCIDENCE. THERE ARE SEVERAL FACTORS WHICH CONTRIBUTED TO THIS EVENT. THE AIR-CONDITIONING SWITCHES WERE TURNED ON AFTER THE ENGS WERE STARTED AND TURNED OFF SO THAT THE ACFT COULD BE DEICED WHILE ON THE GND. AFTER THE DEICE PROC THE SWITCHES WERE NOT TURNED BACK ON. DURING THE DEICE PROC I WAS TRYING TO GET OUR WT AND BAL DATA FROM LOAD CTL. IT TURNED OUT THAT THEY WERE USING THE WRONG ACFT NUMBER AND THAT IS WHY WE WERE UNABLE TO RECEIVE THE WT AND BAL OVER THE ACARS. I CHKED THE FLT RELEASE AND IT HAD THE WRONG ACFT NUMBER ON IT. AS WE TAXIED FROM THE DEICE PAD, I CALLED SYS CTL TO GET AN AMENDED RELEASE. WHILE TALKING TO SYS, THE CAPT KEPT TELLING ME WHAT TO SAY AND SEEMED AGITATED. HE THEN GOT ON THE RADIO AND SAID THAT HE NEEDED AN AMENDED RELEASE IMMEDIATELY AND THAT WE WERE #1 FOR TKOF. THEY GAVE HIM A VERBAL RELEASE ON THE RADIO. THE BEFORE TKOF CHKLIST WAS COMPLETED AND WE WERE CLRED FOR TKOF. THE RECONFIGN CHKLIST WAS NOT COMPLETED AFTER THE DEICE PROC. THERE WAS A LOT OF CONFUSION.
