Narrative:

At departure time, #1 flight attendant notified we had supplemental oxygen on board aircraft. I told her I was not aware of it. We called the agent back to verify the required paperwork was on board since we didn't have any form X. After a quick departure and his return, he assured us we had all the paperwork which was required which amounted only to the #1 flight attendant's paperwork. After reviewing part 1 and the operations manual, we were still not sure that the form X was required, but it stated that cargo and passenger service was responsible, so we took it at that. When landing at dfw, I called cargo and they didn't know the answer, but called and left a message at the hotel to inform me that a form X was required. I feel flight manual part 1 does not clearly address the situation.

Title: PREFLT PAPERWORK NOT COMPLETE.

Narrative: AT DEP TIME, #1 FLT ATTENDANT NOTIFIED WE HAD SUPPLEMENTAL OXYGEN ON BOARD ACFT. I TOLD HER I WAS NOT AWARE OF IT. WE CALLED THE AGENT BACK TO VERIFY THE REQUIRED PAPERWORK WAS ON BOARD SINCE WE DIDN'T HAVE ANY FORM X. AFTER A QUICK DEP AND HIS RETURN, HE ASSURED US WE HAD ALL THE PAPERWORK WHICH WAS REQUIRED WHICH AMOUNTED ONLY TO THE #1 FLT ATTENDANT'S PAPERWORK. AFTER REVIEWING PART 1 AND THE OPS MANUAL, WE WERE STILL NOT SURE THAT THE FORM X WAS REQUIRED, BUT IT STATED THAT CARGO AND PAX SVC WAS RESPONSIBLE, SO WE TOOK IT AT THAT. WHEN LNDG AT DFW, I CALLED CARGO AND THEY DIDN'T KNOW THE ANSWER, BUT CALLED AND LEFT A MESSAGE AT THE HOTEL TO INFORM ME THAT A FORM X WAS REQUIRED. I FEEL FLT MANUAL PART 1 DOES NOT CLRLY ADDRESS THE SIT.
