Narrative:

Flight XXX was dfw.WORTH5.abi.J65.pxr..bkx.J212.psp.civet. Lax unable to get pre departure clearance. Verbal clearance read dfw 262 degree radial and worth intersection. Chism transition J65 as filed. Read back clearance without realizing there is no chism transition. Delay at push due to cabin problem. During taxi out, ground was busy and couldn't select correct runway on FMC. Late load close-out and had trouble with temperature control in forward cabin. After departure, ATC gave us a heading to 'join the SID.' a few moments later, ATC asked if we were joining the 262 degree radial and gave us a turn back to the radial we had already crossed. They stated our clearance was the dfw 262 degree radial to worth, direct chism J65. We were under the impression that 'cleared to join the SID' meant we were to join the dfw 248 degree radial over mqp and abi per our flight plan. During taxi and climb we were distracted with aircraft and FMS difficulties and failed to check and verify an ambiguous departure clearance. Supplemental information from acn 309830: given verbal clearance which stated cleared to lax via dfw 262 degree radial to worth intersection and then via chism transition. First officer and captain confused by chism transition which was not on the worth 5 departure. We should have checked on an ambiguous clearance, but we were distracted by numerous aircraft and takeoff performance system problems.

Title: ACR FLC FAILED TO FOLLOW ASSIGNED SID DEP RESULTING IN ATC INTERVENTION FOR VECTOR TO ASSIGNED TRACK.

Narrative: FLT XXX WAS DFW.WORTH5.ABI.J65.PXR..BKX.J212.PSP.CIVET. LAX UNABLE TO GET PDC. VERBAL CLRNC READ DFW 262 DEG RADIAL AND WORTH INTXN. CHISM TRANSITION J65 AS FILED. READ BACK CLRNC WITHOUT REALIZING THERE IS NO CHISM TRANSITION. DELAY AT PUSH DUE TO CABIN PROB. DURING TAXI OUT, GND WAS BUSY AND COULDN'T SELECT CORRECT RWY ON FMC. LATE LOAD CLOSE-OUT AND HAD TROUBLE WITH TEMP CTL IN FORWARD CABIN. AFTER DEP, ATC GAVE US A HDG TO 'JOIN THE SID.' A FEW MOMENTS LATER, ATC ASKED IF WE WERE JOINING THE 262 DEG RADIAL AND GAVE US A TURN BACK TO THE RADIAL WE HAD ALREADY CROSSED. THEY STATED OUR CLRNC WAS THE DFW 262 DEG RADIAL TO WORTH, DIRECT CHISM J65. WE WERE UNDER THE IMPRESSION THAT 'CLRED TO JOIN THE SID' MEANT WE WERE TO JOIN THE DFW 248 DEG RADIAL OVER MQP AND ABI PER OUR FLT PLAN. DURING TAXI AND CLB WE WERE DISTRACTED WITH ACFT AND FMS DIFFICULTIES AND FAILED TO CHK AND VERIFY AN AMBIGUOUS DEP CLRNC. SUPPLEMENTAL INFO FROM ACN 309830: GIVEN VERBAL CLRNC WHICH STATED CLRED TO LAX VIA DFW 262 DEG RADIAL TO WORTH INTXN AND THEN VIA CHISM TRANSITION. FO AND CAPT CONFUSED BY CHISM TRANSITION WHICH WAS NOT ON THE WORTH 5 DEP. WE SHOULD HAVE CHKED ON AN AMBIGUOUS CLRNC, BUT WE WERE DISTRACTED BY NUMEROUS ACFT AND TKOF PERFORMANCE SYS PROBS.
