Narrative:

Inbound to dca -- cockpit temperature controller ran away, full hot 130 degree F. Unable to control with temperature selector or trim air. Pulled and reset breakers, cleared problem, wrote up. Dca mechanics said there was a history on plane that right pack needed to be on MEL due to bite check. Called moc to double check MEL because cockpit is on left pack. Said ok. Departed to south from dca and right bleed tripped several times and we reset. Cabin climbing 1000 FPM -- passed through 5000 ft and unable to control cabin altitude -- went to standby and manual and closed outflow valve. Still climbing. Asked for lower or level off at 9000 ft. ATC said we must keep climbing to 11000 ft due to traffic. Leveled at 11000 ft. Cabin altitude horn went off at 10000 ft, reset -- cabin leveled off at 11000 ft and we were cleared for a slow descent into dca 500 FPM. Landed and checked east and east doors shut. Suspect forward outflow valve never closed at 2.5 psi.

CABIN ALT HORN WENT OFF AT 10000 FT, RESET -- CABIN LEVELED OFF AT 11000 FT AND WE WERE CLRED FOR A SLOW DSCNT INTO DCA 500 FPM. LANDED AND CHKED E AND E DOORS SHUT.
