Narrative:

On jun/xx/95, dc-9 aircraft blew the left outboard main landing gear tire on takeoff from dca. The aircraft diverted to bwi without incident. Examination of the blown tire revealed that it was of the incorrect ply rating. The correct ply rating for a dc-9 is 24 ply. The tire that blew-out was a 16 ply.further investigation at the wheel shop in gso revealed a tire for a boeing 737 with the incorrect ply rating on the tread. The ply rating on the tread was 24 and the ply rating on the tire carcass was 26. Also, investigation revealed a tire for a dc-9 in the tire slot for a F-28. To insure that this type of error does not occur again, the wheel shop now has a policy that requires the mechanic that torques the wheel bolts to mark with a 'check mark' the tire size and ply rating on both the tire carcass and the tire re-tread. Also, the wheel build-up will be stamped to that effect.

Narrative: ON JUN/XX/95, DC-9 ACFT BLEW THE L OUTBOARD MAIN LNDG GEAR TIRE ON TKOF FROM DCA. THE ACFT DIVERTED TO BWI WITHOUT INCIDENT. THE PLY RATING ON THE TREAD WAS 24 AND THE PLY RATING ON THE TIRE CARCASS WAS 26. ALSO, INVESTIGATION REVEALED A TIRE FOR A DC-9 IN THE TIRE SLOT FOR A F-28. TO INSURE THAT THIS TYPE OF ERROR DOES NOT OCCUR AGAIN, THE WHEEL SHOP NOW HAS A POLICY THAT REQUIRES THE MECH THAT TORQUES THE WHEEL BOLTS TO MARK WITH A 'CHK MARK' THE TIRE SIZE AND PLY RATING ON BOTH THE TIRE CARCASS AND THE TIRE RE-TREAD.
