Narrative:

Prior to departure, I received the air carrier form OK333 for dangerous goods/hazardous materials notification. Everything on the form was correct. However, once again, the final load closeout did not have a restricted article annotated. This is not the first time this has happened. 1 of many. It was not until much later in the flight that we noticed the code RA was missing from the load closeout. Had there been an emergency of any kind, no one, especially ftw dispatch would have known that this material was on board. There simply must be a way to get these load closeout's accurate. Our airport analysis specifically states that this annotation must be on the load closeout so that everyone involved with the flight knows exactly what is on each and every aircraft. En route to las, I phone patched with ftw on a different matter and did mention that we had hazardous material on board.

Title: AN ACR MLG CAPT FOUND THAT NOT ALL OF HIS PAPERWORK WAS IN ORDER.

Narrative: PRIOR TO DEP, I RECEIVED THE ACR FORM OK333 FOR DANGEROUS GOODS/HAZARDOUS MATERIALS NOTIFICATION. EVERYTHING ON THE FORM WAS CORRECT. HOWEVER, ONCE AGAIN, THE FINAL LOAD CLOSEOUT DID NOT HAVE A RESTRICTED ARTICLE ANNOTATED. THIS IS NOT THE FIRST TIME THIS HAS HAPPENED. 1 OF MANY. IT WAS NOT UNTIL MUCH LATER IN THE FLT THAT WE NOTICED THE CODE RA WAS MISSING FROM THE LOAD CLOSEOUT. HAD THERE BEEN AN EMER OF ANY KIND, NO ONE, ESPECIALLY FTW DISPATCH WOULD HAVE KNOWN THAT THIS MATERIAL WAS ON BOARD. THERE SIMPLY MUST BE A WAY TO GET THESE LOAD CLOSEOUT'S ACCURATE. OUR ARPT ANALYSIS SPECIFICALLY STATES THAT THIS ANNOTATION MUST BE ON THE LOAD CLOSEOUT SO THAT EVERYONE INVOLVED WITH THE FLT KNOWS EXACTLY WHAT IS ON EACH AND EVERY ACFT. ENRTE TO LAS, I PHONE PATCHED WITH FTW ON A DIFFERENT MATTER AND DID MENTION THAT WE HAD HAZARDOUS MATERIAL ON BOARD.
